Paradoxical changes in bispectral index during nitrous oxide administration.
I report two patients undergoing open heart surgery, with nitrous oxide and isoflurane anaesthesia, for whom bispectral index (BIS) monitoring showed high BIS values with nitrous oxide and isoflurane anaesthesia. The BIS decreased immediately after nitrous oxide was stopped and increased again after nitrous oxide was restarted.

OBJECTIVES To determine whether bispectral analysis (BIS) changes during nitrous oxide (N(2)O) sedation in anxious children undergoing extraction of primary teeth. METHODS In this prospective study 45, ASA physical status I children, aged between 7 to 12 years and scheduled for primary teeth extraction under N(2)O/O(2) sedation are included. At baseline (T0) and during the sedation procedure ...

EDITOR: Several authors have reported sudden increases of the bispectral index values during different procedures such as endoscopic shoulder surgery [1], deep hypothermic circulatory arrest [2] or after using an upper-body blanket for warming [3]. In addition, spurious bispectral index values can be observed when the electromyogram activity of the bispectral index monitor reaches a value of 50...

was erosion of the patient’s nasogastric tube through the anterior esophageal wall into the trachea. The finding that the defect was larger on the esophageal side than on the tracheal side makes bronchoscopy an unlikely cause. None of the other common causes of TEF in adults (radiation therapy, trauma, corrosive burn, malignancy, congenital defect, or tuberculosis) apply to this patient. There ...

EDITORIALS 211 inotropic agents (such as the catecholamines, calcium ions, and digitalis), and paired electrical stimulation. Entropy also increases during congestive heart failure, which may be thought of as an expression of a defect in the utilization of energy, a partial chemicomechanical uncoupling. The entropy of the myocardial cell tends to be reduced by negative inotropic agents, such as...
